Oregon has its own W4 starting in 2019

Changes to the tax laws at the federal level may result in Oregon workers not having enough taxes taken from their paychecks to cover their state liability come tax time. In an effort to encourage employees to ensure they have enough taxes withheld, Oregon now requires its own state specific W-4 as the preferred method for collecting state withholdings.

The best practice for compliance is to include this form any time you have an employee fill out the federal W-4 form.
